You directly provide the company with the data we collect. We collect data and process data when you:

B)   complete a patient registration form at the hospital

A)   contact you to schedule clinic appointments and arrange tests

B)   provide necessary clinical information for your general practitioner and other health professionals for medical safety and for the implementation of safe medical treatment.

Our company securely stores your data on an encrypted data base at a secure location. Medical notes will be kept for 25 years. Paper notes are kept in locked filing cabinets at a secure location. Your data will be deleted permanently from the data base after that time.

We will only contact you to schedule necessary clinical appointments. We will not use it to try to sell you other products and services, or share it with a third party for similar purposes.
